Before you begin, please have the following ready:
- Student ID numbers for each student who attends the school
- Names of children who live in the household but do not attend the school
- Last 4 digits of Social Security Number of the certifying adult (if available)
- Income information for all household members
- Case numbers for SNAP, TANF, or FDPIR (if applicable)
Remember: include all children who live in your household, even if they do not attend this school.
